What is a Fit Note?
A fit note (sometimes called a sick note) is a statement from a healthcare professional confirming that you are not fit for work, or may be fit for work with support from your employer.
When Do I Need a Fit Note?
- You can self-certify for the first 7 calendar days of illness.
- A fit note is usually only required if your illness lasts longer than 7 calendar days.
- Your employer should accept self-certification during the first 7 days.
How to Request a Fit Note
- Submit an online consultation/request.
- Telephone the surgery.
- Visit reception.
Requests are reviewed by a clinician and are not issued instantly. An appointment or telephone assessment may be required.
Renewing a Fit Note
- Request an extension up to 3 days before your current fit note expires.
- Earlier requests may not be processed.
- Please keep your employer informed while waiting for a decision.
Returning to Work
- If you recover before your fit note expires, you can return to work without needing a “fit to return” note from the surgery.
- Discuss your return with your employer.
