New Patients

Providing you live within our Practice area, you can register with us by completing a Registration Form and patients aged 16 and over must complete the New Patient Questionnaire. Patients under aged 16 need to complete the Under age 16 questionnaire Once completed, bring them in, with a form of ID and hand to the reception desk. You will be registered within two working days, then your records will be transferred to us by the Health Authority from your previous practice.

If you’re previous practice was outside of England please also complete this form.

If you are on any repeat medication, please book a medication review with our Practice Pharmacist. We also encourage new patients to book an NHS Health check (if you haven’t had one at your previous practice in the last 5 years).

All new patients are registered for online access, where you can manage your appointments, order your repeat medication and view prospective data on your medical record. Please complete the prospective data form. You can opt out of this service on the New Patient Questionnaire.

Please note you WILL NOT be registered at the Practice unless you complete a New Patient Questionnaire and Registration Form.

Registered patients who change address and stay within our practice boundary are required to complete a change of address form and provide a copy of a utility bill or council tax statement showing their new address.
